9th GRADERS/NEW STUDENTS/TRANSFER STUDENTS:

MEDICATIONS:

  • All PRESCRIPTION MEDICATION must be accompanied by written permission from both a parent/guardian and physician. There are separate forms for over the counter, prescription and emergency medications.
  • Prescription medication shall be brought to the school by the parent/guardian unless other arrangements have been made with the nurse. It must be in the original prescription container, labeled with the name of the student, medication, dosage and name of the physician. All medication is to be taken home when it is no longer needed or at the end of the school year.
  • Please submit the appropriate action plan for student conditions (ie, emergency epinephrine, asthma, diabetes, seizures, adrenal insufficiency, etc.) each year.  

SPORTS:

  • Students interested in playing a sport must submit a current Pre-participation physical and any appropriate action plans and register in Family ID at least two weeks before the announced sport deadlines. Any student who is out of gym due to injury must be cleared by their physician and return to gym before they can participate in a sport.

SCREENINGS:

The following screenings are required by the state and will be conducted in school by the certified school nurses.

  • Grade 9 height, weight, blood pressure and vision
  • Grade 10 height, weight and blood pressure
  • Grade 11 height, weight, blood pressure scoliosis and hearing
  • Grade 12 height, weight and blood pressure

The school district shall provide for the notification of the parent of any student suspected of deviation from the recommended standards.
